2. He provided anesthesia during the cystoscopy performed on Reagan on Aug. 9, 1986 (with Dr. Stephen Rettke).
 

3. He was probably part of the team that performed Reagan's prostate operation on Jan. 5, 1987.
 

4. He also provided anesthesia during Mrs. Reagan's breast cancer operation on Oct. 17, 1987.
